a question about a pdf file

This is specifically for those who can see, and for anyone else who wants to try. This file is from a college website. I need to fill it out. It doesn't appear to be editable. Can this be edited without a screen reader? Here's the link to the file on my one drive.

That link isn't working for me. I suspect it is a "paper form" being electronically distributed where you are expected to print it out and fill it out as Gene suggests.
