Insert + space bar toggles browse mode on and off.  It never fails me.

If I have a page in Fierfox open and close NVDA, the next time I start NVDA it appears to get stuck in focus mode so I can't easily navigate around the page. I have to restart Firefox to fix this. Anyone know a solution?
