I suspect in that case a locked file. Windows is good at doing this for no apparent reason, maybe nvdas updater/installer needs a timeout feature to stop this kind of hiccup.

Many peeices of software do suggest making sure all things are closed before you do an install in case a dll is flagged as in use and hence it gets stuck or asks to reboot after the install.
