Really the actual document has to have been made interactive in the first place for this to work. The supplier has probably just sent a pdf for sighted people to print out and fill in.
You can either ask them to rectify this either by an on line html form or supply you with a document you can edit.
I've seen this so many times from companies who really should know better.
I guess you might be able to extract the text and then reformat it into a text document, but why should you have to do their work for them?

I was wondering if there is a way to fill out forms in adobe reader. These forms don't actually have any clear indications of checkboxes or anything like that that. This form just has ___ where you are supposed to put your information.
