You do need to have an account on GitHub to create an issue, particularly so it can alert you when anyone else comments or updates any issues you create or subscribe to. It wasn't too invasive in the details it wanted last time I checked, but do let me know if that's changed. Once you have an account and have signed in, you can go to: https://github.com/nvaccess/nvda/issues to browse or search existing issues (actually you can do that without an account) and https://github.com/nvaccess/nvda/issues/new to create a new issue.
