The level setting, some most all, etc. has nothing to do with how the symbol is spoken.  It only affects where it is spoken. 
 
I'm testing with an old version of NVDA so this may not apply any longer.  I am testing the portable version.  But there appears to be a bug in the symbol pronunciation function.  I changed the pronunciation of exclamation point to exclaim.  I changed the when spoken setting to character.  I then tried to change it back to bang and set the when spoken setting to all.  It remains stuck on saying exclaim.  I've tried making the change with different setting levels set before I made the change and changed the settings level afterwords.  Nothing I do changes what is spoken.  It still says exclaim.  I also tried leaving the pronounced as field blank.  Even then, when I read a sentence or even the word itself written on a single line in Notepad, it still says exclaim.  I assume it will do this wherever the sign is encountered.  Unless a bug was found and corrected at some point related to the punctuation mark pronunciation feature, it needs to be tested rigorously so that others don't have things get stuck as I did.
