And it is this part which has stopped working on my machine. Everything is totally silent but does in fact change. also of course the same issues exist in share and log in as well.

Something in Windows has broken but nobody seems to know what. I strongly suspect Avast was the culprit when it was removed, but since i never thought to check back then, I have no way to get back without doing a complete restore from back ups and probably losing a lot of stuff and screwing stuff up. I think if there was a command to re register all the existing dll files, then that is likely to fix it. of course since windows throws no errors its hard to diagnose. Nvda merely seems to not be able to see it in the logs as one might expect. Jaws demo does exactly the same it announces the dropbox preferences title and the share title but can see nothing in the open window.
