There are logs or information Windows keeps about crashes.  I’m not sure how you find it.  But if some of the information isn’t too technical, you can tell what program is crashing.  At times, a program can crash in such a way that it causes NVDA to stop speaking even if you try to move outside of the program window.  I don’t know why.
