What the user wants is that NVDA announces automatically the phonetic alphabet.

 

He must use this add-on:

Author: David CM
Add-on name: Enhanced Phonetic Reading
Description: This add-on add some features to phonetic reading as the following:
announce the character description for the last read character after n milliseconds when the character was read by character review commands, E.G. left and right arrow keys, left, current or right character review commands, ETC.
Read the character description instead of the current character. This feature must be enabled or disabled manually and will be deactivated when NVDA is closed.

NVDA can give you phonetic pronunciation, but the execution is slightly different.  If you press many NVDA keystrokes once, they will read the information, twice quickly spells it, and three times quickly spells it phonetically.  For instance, if you press NVDA+up arrow (or NVDA+l if using laptop keyboard layout), NVDA will read the current line.  If you press it twice quickly, it will spell the current line, and if you press it three times quickly it will spell it phonetically.

(For those thinking ahead, yes they report the word or character at the review cursor, but anyone not already familiar with that probably isn't moving the review cursor independently of the caret too often).
