NVDA not focusing to desktop when closing apps: it depends on where NVDA thinks system focus is. NVDA stores a reference to object with system focus, and when a program closes, NVDA will move focus to where it thinks system focus is.

As for NVDA saying "explorer" when you perform title (NVDA+T) command: this is because NVDA learns that there is no label for the foreground window. When this happens, NVDA will resort to announcing the executable name of the program you are focused on, so NVDA will announce "explorer" since it is the executable name of File Explorer (explorer.exe). In fact, when you press Windows+D to show desktop, NVDA will say "explorer" even if "Program Manager" is present but NVDA sees it as "unknown".
