eSpeak is built into NVDA! You don't need to do anything to use it. However, there is a SAPI 5 version of it but I see no need to use this unless you want to use it with other programs that have speech.

Years ago there were a few free addons to add extra voices to NVDA. However,
as NVDA evolved and became incompatible with older addons those addons never
updated their software. ESpeak may be one of the last, no-cost synthesizers
and is still one that I fall back to. Of course, you have Microsoft's voices
which have improved and have become more responsive.

I think there are extra voices for older versions of NVDA. Do a search for
extra voices for NVDA on either google or any other search engine you use.
