Well, additional developments that some may find interesting.  I downloaded the tax year 2015 (as in should have been filed on April 18th 2016) version of the 1040EZ and I still get the same result in Adobe Reader DC.

So, I decided to try a form that I'd customized in Microsoft Word for a client about a year and a half ago where every blessed field (or at least the ones I set up in the demo form - I didn't complete the myriad checkboxes that the paper version contains) has alternate text and help text.  If I open this in Word and have NVDA running it does precisely what I think it should do:  Announces the alternate text when I land in the form field and, if I hit F1, displays the help text box and reads its contents.

I have to believe that either the IRS forms are not set up correctly to be accessible or else there's something that doesn't "play well together" between Adobe Reader DC and NVDA.

If I hit NVDA+TAB in anything I've tried so far I do not get the field name announced.

Dexter Not Duplicate threat it is the exact same message twice but it does not happen on every single message but almost every other message that I get a duplicate from the same person with the same message

This is a known issue, and a fix is on its way. In NVDA 2016.2, the
chart sheets will become accessible. If you don't want to wait a month
for the fix, you can download a Master snapshot of NVDA.

Windows own audio settings? As in, change the input channel it listens to?

Sorry can't be of more help, but, really just used it for simple little tests. Virtual recorder does, alternatively give you a bit more control over what it will listen to/use as recording source?

You'd then still need to patch the audio over the video clip though, using virtual dub, or something similar?

Been a little while since last really used it, but, think it just recorded system audio output - you could set system audio to listen to microphone, but, remember main thing was extracting audio from video clip using audacity, tweaking it a bit, and reassigning it to video clip using virtual dub, and then reconverting that video clip to something a lot smaller - ongoing issue with virtualDub is trying to get it to use alternative encoders, but anyway.

Just fired up webInAria, and if read dialogue/window, see it offers setting to record microphone, but, not sure that would then include actual system output as well, and, yes, the interface isn't perfect, but, like said, like that it offered global hotkeys to start, pause and stop recording.

Yeah surprisingly it was not hard. Flummoxed if you have VS 2015
community as it and windows SDK 7.1a SDK don't play at all nice with
each other, so I removed it and installed VS 2015 express, I don't need

The message states that tabbing doesn't work.  If tabbing doesn't work, it is very unlikely that numpad 5 or, what is actually the correct key, numpad 8, will work.  Numpad 8 is the read current line command.  Numpad 5 is the read current word command.  You want to read the line, not a single word. 
 
The only command I know that may be relevant is NVDA key tab. JAWS key tab does the same thing.  It reads the field you are in.  While possible that this will help, I doubt it.  but you can try.

It may correct itself.  It may happen on any list and it may then stop.  I don't know the technical reasons that may cause such a problem.
