Can't help with the first question, but try hitting the key combo nvda key plus 4. This will speak command keys on which includes the enter key so it will speak that when you use it.

Did you hit Control Y to bring up the list of folders? After that, you should be able to use your arrow keys to move around the folder list.

When you say it "renders" in browse mode.....does NVDA read the captions in a series, or are you able to click through them one by one and then select one using the enter key? I don't have

I would think using another screen-reader wouldn't be necessary. You can probably cause NVDA to read the document in browse mode by simply pressing alt one time to move to the ribbons and alt once
