One of the issues I've seen in this regard is if the screen scrolls or the system is a little laggy for whatever reason the wrong line is read.

I'm not at all sure why NVDA up arrow is reading the wrong line, but try making sure you are using object review and see if that helps. do you know the commands to change among the different review

First, to have NVDA saying just the icon name and not the first letter of it, press NVDA+F2.
You should hear NVDA say "Speak typed characters off".

I always use the custom install and uncheck the visualizations, quick launch icon (which allows Winamp to appear in the system tray) as well as the modern skin

When I’m in a tree view looking for a document, I type the letter “D: to get to Desktop or Dropbox. NVDA announces “D: Desktop or “D: Dropbox, depending on how many times I’ve hit the letter

I am running v14 over here with out any issues
now version 15 on the other hand you do have to use jaws cursor to accewss the minue items, have no idea about nvda with it.
