I don't know if the bug was ever corrected, but Omnipage used to and may still have a bug that causes text to be misplaced when scanning multiple columns or two opposing pages. Fine Reader never had

They have removed all submenus in the compilation menu. You just arrow down to which ever you need and enter. Alt d to add files and control b to burn.

It looked like it happened in Firefox. Are you using the Mozilla apps add-on? If so is it the latest version and / or have you tried with the add-on disabled? I know I've seen this loss of focus

ABBYY was updated last year. I haven’t yet purchased the upgrade, so can’t comment on the latest version. It handles document formatting well, including headings and footnotes from scanned images
