Does a sighted person see some kind of indication that this is happening?
I would suggest the speech history add-on. It's a bit more convenient than the speech viewer.

No, there is no way. I’m wondering why you ask for it. Purpose of opening a mail is to read it….at least for me.
But feel free to log an issue with NVDA.

I don't see any way, but why does it matter?
Whenever I open anything, a program, a document, something is spoken. When I open a document in Notepad, for example, the current line is spoken.

I do get all three of these issues, but they are of minor concern to me. The one thing you didn't mention is the reading of the underlying address when you tab to or arrow to a clickable
