Did you try doing a restart? I have to do that from time to time when that happens.

I believe this might have been mentioned before, but sometimes when pasting text using the clipboard history menu, either the control key will stick, or it'll just insert

Yes, trouble is, I can’t get to the menu with insert+n and then preferences.

If you have a windows key, then windows key + B to focus on system tray/notifications bar, and, then look for the NVDA icon there - if it's not showing, you might need to first shift + tab

Hello,
Go to the NVDA icon in the systray by pressing Windows-B (on your Mac keyboard Option-B), hit the letter N until you come to NVDA and press Enter

I should have added that I am using Windows 7 and using NVDA as a portable program, and that may or may not make a difference.

I was going to say, if you had a way of reliably reproducing it, you could try disabling add-ons, then do that and see if it happens again - but if it doesn't happen all
