It doesn’t say cap when using reade to end or reading line by line. It doesn’t even say cap when moving by word. In short, you get the same information as you do with pitch change. I agree that

I've always found the pitch change to be barely audible even when the level is ramped up! The problem with the "Say Cap" and "Beep for Caps" is that it doesn't do it when you are using Read All, only

This sort of thing is virtually always an issue with the synth, so it would be worth reporting to the synth maker with your example text, too.

Did you place the entries in the dictionary at the same time or whenever you did, are they both still there? Remove the American Samoa one, which is not correct. That entry tells NVDA to say
