I guess that under tools in the kindle program there is a text to speech related item, if it isn't available from there you should check the preferences sections under tools menu.

CTRL+T will turn off the built-in TTS.
Note that not all books from Amazon support Screen Readers.

I've used the Eloquence/Vocalizer addon with beta and RC builds with no difficulties. Of course, this assumes that you're not working with a beta which breaks compatibility with the addon.
