I knew they had disabled the functionality back in windows 7 but did not realize how long ago it was.
In the machines i use now the run or install option under the auto play menu is disabled

Here is an article from Microsoft discussing changes made to Windows 7 at some point after its release. Autorun was disabled, autoplay wasn't.

If i remember right they disabled the autoplay menu so it did not work
any more.
I know what you are talking about it was the first option in the
autoplay menu and you just pushed the enter

When I first started to use NVDA, I put it on a thumb drive in its own folder but had an "autorun" file in the root. Plugging in the drive made the system execute the commands in the autorun file

usually when i do a portable version of nvda i usually put it into a
folder called nvda and all of its files go into that. it means i can
also have other folders and keep it better structured

If you are not sure where to get the official add ons from when nvda is running press the nvda key + letter N a menu will pop up then arrow down to tools then a sub menu will appear then arrow
