I have used one note and it's ok, they suggest hitting f6 to get between pains. I submitted feedback a while ago, and that's as far as it went.

Nvda+alt+UP or down arrows scroll through the time zones in the ring
Nvda+alt+T will read the last time zone then
And reading the documentation will make everything clear

I read the original request as Gene did. I thought it might be possible to control this either with punctuation level or possibly by adding a regular expression match for a blank line in the default

Because it does not solve the problem totally. For example: what if I want to read the previous paragraph, but there is a lot of blank lines between the current and the previous paragraph?

Use the audio ring, NVDA+Control+Left/right arrow loops around various audio features, one of which is voice. Once set on Voice, use NVDA+Control+Up/Down arrow to switch between voices.
