How do you know there is no window? Do you have enough vision to know? Have you looked through the desktop to see if it is read normally? Have you opened a program, not maximized but normalized and

How about checking your keyboard layout. If using Windows 10, cycle with Windows+space and see if you haven't unintentionally changed layout. Left alt+shift also worked on older OS's.

A hovering window shouldn't really affect the system tray context menu of NVDA, and, if you firstly hit either windows key + M, or windows key + D, before trying out the ctrl + alt + N

Are you saying you hear these letters when you down arrow through what should be menus or do you hear them read immediately after issuing insert n with you taking no further actions? When you press

I doubt it's the keyboard.
The first thing I would do is reboot. If that doesn't solve the problem, at least part of it may be that there is a window covering part of the screen. If you have

If you are able to activate input help - NVDA key + numer row 1 - then you could check what keystrokes are being passed to NVDA/windows?
As in, almost sounds like there are incorrect keystrokes

I am using a wireless keyboard usb and everything works as all of the keys work as they should. this is the only keyboard that I have.

Sounds like a problem keyboard. Do you have an external keyboard you can plug in to see if that works OK?
