Some VMware tips.
First, VMware does some mapping of the keys to give you more mac-like functionality (e.g. cmd+c for copy, etc). This can be disabled in the preferences, I think it's called keyboard

Then Maybe I'm wrong. If right alt worked before, maybe there is a way to do it again. But since you haven't been able to do it, perhaps there is some slight difference in your installation of

When you define a key, it is, assumedly, sending exactly the same code to the system as any other key you define. My guess is that, if you can't use insert or caps lock, you can't use anything. it's

You can remap some keys to be used as Insert.
One of my friends have remapped F3 to Insert...

I can find nothing in the documentation that suggests or states that you can use anything other than the three usual (num pad insert, regular insert, caps lock) choices as the NVDA modifier key.

Just hit NVDA+shift+p when the focus is on the application you want to
have a dictionary for, and the normal NVDA dictionary dialog will pop
up for you to add entries valid only for this

I realize that, but that's what vmWare fusion won't me do.
I can't use capslock as my NVDA modifyer, under vmWare fusion.

You could designate the caps lock key as your NVDA key instead of using the insert key.
