Are you using object navigation or flat review? I think it's called screen review in newer versions of NVDA. Use object navigation. Move using numpad insert 4 to go baqck and nunmpad insert 6 to go

Use the review cursor to read the previous text to get the misspelled word, and you can also read forward further down the pane to get the dictionary meaning of replacement words. To read the context
