I believe I've also seen this. It will sometiems go away on its own, and sometimes not. Hagve you reported this to github so they can keep track of this as well?

If you happen to know Linux/Unix, the awk command will be very easy to use to do the conversion.
A JAWS dictionary line has the fields, in order: Actual word|Replacement

I don't have any SAPI 4 voices anymore to check.

By using Narrator I have did above settings which you have mention.
But Microsoft AP 4 is not working.
Now reinstall NVDA and change the synthesizer from E-speak to
Microsoft AP 4.
