Are you referring to the COM registration fixing tool? If so, I'm pretty sure that all it does it write CLS IDs to the registry for accessibility related DLLs.

That's probably a synthesizer issue, as far as I know NVDA doesn't attempt to distinguish between the various types of dots other than the sentence ending and the symbol.

Personally I think that dialog should probably not save the response to it in nvda.ini and should be a separate configuration file because of this.

Troubles with the last update
There was an issue that caused a beta to be inadvertently offered to stable users. Just redownload 2018.2.1 and that sound should go away.

Yes, go in to settings>time and language>speech and set the rate there, and NVDA's speech rate will increase along with it. This is unfortunately out of NVDA's control.
