braille on user account control screens?

Yes, I did save the settings, and that was my concern, is that it saves the speech settings but doesn't give braille everywhere. I'll check and see what other screen readers do to work with this, but I was just thinking it possibly might be a problem for some.

Can't be done.  I griped about this a while back.
I doubt anybody totally deafblind can use nvda.

In the 'Preferences > General Settings', have you tried pressing the
'Use currently saved settings on the logon and other secure screens'
button?
