I'm not at all clear on what you're asking.

Go to the NVDA menu, to Preferences, then Settings. Arrow down to Document settings, and press Tab. There, you should find options to have NVDA read you whatever formatting you want. Most of these are checkboxes. You can tab through them and press the spacebar to select the ones you want. When you're done, press the OK button. I hope that helps.

Can you be clearer of what your use case is? This way we can help you to troubleshoot a bit easier. You want to read underline words, but now? In say all, word by word? Etc.?

Here, again, we have a lack of specificity. What "it" is being referred to?
Others have already mentioned the way to bring up NVDA's Document Formatting settings. Every NVDA user needs to know how to do two basic things when almost any question arises. The one you want really depends on the exact nature of the question: 1. Bring up the NVDA Commands Quick Reference: NVDA + N, H, Q [NVDA Menu, Help, Commands Quick Reference] 2.
