I'm not at all clear on what you're asking.

That being said, go to document formatting settings in NVDA and you can select what gets reported to you as far as formats applied when they're encountered.

Go to the NVDA menu, to Preferences, then Settings. Arrow down to Document settings, and press Tab. There, you should find options to have NVDA read you whatever formatting you want. Most of these are checkboxes. You can tab through them and press the spacebar to select the ones you want. When you're done, press the OK button. I hope that helps.

Can you be clearer of what your use case is? This way we can help you to troubleshoot a bit easier. You want to read underline words, but now? In say all, word by word? Etc.?

What happens when you press CTRL+NVDA+D, then explore with Tab and Shift+Tab?

Here, again, we have a lack of specificity.  What "it" is being referred to?

Others have already mentioned the way to bring up NVDA's Document Formatting settings.  Every NVDA user needs to know how to do two basic things when almost any question arises.  The one you want really depends on the exact nature of the question:

1.        Bring up the NVDA Commands Quick Reference:  NVDA + N, H, Q  [NVDA Menu, Help, Commands Quick Reference]

2.      Bring up the NVDA User Guide:  NVDA + N, H, U   [NVDA Menu, Help, User Guide]

The details regarding document formatting settings are in the User Guide and can be found by searching on that exact phrase.  The Font Attributes checkbox should cover bold, italic, and underline attributes.

Sarah is absolutely right that an example of what's wanted would go a long way in getting the correct answer.
